 Treatment of angina
If strep throat is essential to take proper antibiotic treatment, to prevent the occurrence of complications. If sore throat is viral, antibiotics are useless instead. A simple test can tell the difference between these two types of angina in the office of general.
The treatment of angina has two aims: firstly relieve symptoms associated with infection of the tonsils and pharynx (pain on swallowing, fever ...) and secondly, prevent complications that can lead to angina of bacterial origin. As a result, the management will be quite different depending on whether angina is due to a viral infection (the most frequent case) or a bacterial infection (for beta-hemolytic streptococcus group A common cause of bacterial tonsillitis). In the first case, symptomatic treatment is sufficient. In the second, antibiotics are needed.
Angina: A test to identify the cause of the infection
In recent years doctors have rapid diagnostic tests (RDTs) of angina to group A streptococcal Easy to use, they can learn in 5 minutes if angina is related to strep. For this, the doctor performs a collection, from a swab (stick covered with cotton) on the amygdala and the sample immersed in a reagent.
Viral pharyngitis: a question of comfort
When angina is viral or when the TDR is negative, the treatment involves a drug against pain and fever. Paracetamol is preferable, because it is better tolerated, but it is also possible to take an anti-inflammatory drug (NSAID) or aspirin, knowing that it is not recommended in children when viral illness because of complications, exceptional, have been described (Reye syndrome).
These drugs are intended only to improve comfort, until the symptoms disappear by themselves. They can be arrested as soon as symptoms subside. Remember that antibiotics are inactive against viruses. So there is no incentive to consume. These viral pharyngitis heal spontaneously within days.
Strep Throat Symptoms: Follow the doctor's advices!
When you got  strep throat symptoms, you should follow the doctor's advices. When the TDR is positive, this indicates that angina is due to strep. Antibiotic treatment is indicated, in addition to symptomatic treatment, to reduce the risk of complications (including abscess of the tonsil) and prevent transmission of infection to the environment. Antibiotics shorten the duration of angina. The treatments are shorter than before and most often involve a penicillin (amoxicillin) for 6 days. If allergic to penicillin, a cephalosporin, can be used, or a macrolide. It is essential to follow the prescribed treatment to the end, even if symptoms have disappeared, because the goal is to eradicate the streptococcus. If symptoms persist after three days, return visit for a possible change of treatment.
In exceptional cases, a negative RDT must be confirmed by culturing the sample, to verify the absence of streptococci. This precaution is necessary for people who have had rheumatic fever (very rare but serious complication of streptococcal infections) for patients aged 5 to 25 returning from countries where the risk of rheumatic fever is high (Africa, Dom-Tom) and those with recurrent streptococcal tonsillitis.
